SSA Online Social Security Disability Income/SSDI:

 

This allows the Social Security Administration(SSA),   to complete,  and assess your probability for working credits. For ex., an accident, injury, mental health, whether due to work conditions or not.  Not limiting, verifying your interest(s), in applying for Social Security Income (SSI).

Requirements are the following:

-. 18 years of age or/and older

-   Worked jobs covered by SSA

-   Medical condition(s) meet the defined definitions of SSA

If you stopped working due to a physical or mental conditions or both, the following are required:

- The last 5 years from the date of diagnosis of whom you worked for (start date - end date)

 - Names of work corporations, where you were fired due to hospitalizations due to your condition(physical/mental)

 - Fired due to vocal outbursts or arguments

Social Security Income Application/SSI:

 

This application,  is used specifically for applying for SSI. Therefore, requiring only the following:

​

 18 years old or older

 Citizen or a Natural Citizen or a non-citizen who is a qualified alien or in one of certain alien classifications granted by the Department of Homeland Security(DHS).

Disability defined under the terms of the Social Security Administration (SSA)

Results in the inability to do any substantial gainful employment and

-Can be expected to result in death; or

Has lasted or can be expected to last for a continuous period of not less than 12 months.

Compassionate Allowances (CAL) are a way to quickly identify diseases and other medical conditions that, by definition, meet Social Security’s standards for disability benefits. These conditions primarily include certain cancers, adult brain disorders, and a number of rare disorders that affect children and adults.

Social Security Income Application/SSI: Children

 

A person who is neither married (as determined by Social Security) nor head of a household and

​

Under age 18; or

Results marked and severe functional limitations; and

Can be expected to result in death; or

Has lasted or can be expected to last for a continuous period of not less than 12 months.

Original Documents

Once your application has been physically received to the assigned local Social Security Administration, you will be sent a form to sign attesting that an online application has been completed, with your approval, and all the information, is correct. Next you should receive a phone call within 10 business days or less to attest, that another application has been completed. Thereafter, during the application process, you will be asked to either mail or bring in original documents, for ex., lease, birth, marriage, divorce certificates, among others. We suggest you take them in to the local SSA office within your community.

IMA Group

If the doctors' at DDS, need additional information, the IMA will be contacted, and the client/claimant will be scheduled for an in-person appointment (can be more than 1 appointment within the same day or month). If you need to reschedule, please call (518) 389-1310.

At the initial appointment   you MUST have completed the Intake form that specifics the following:
-  The initial Month/Year the you were medically diagnosed with the condition(s)

-  The last 5 years of hospitals, primary care doctors, medical specialists,  physical therapists, private program for abstinence-based recovery, case management programs, shelters, urgent cares, long term hospitalizations, short term visits,  or others.

-  Names of Prison Facilities

- Letters from family, case managers, school records, that can attest to your physical/mental conditions.

All information requires the Names and the Month/Year

Common Reasons for Non-Medical Denial
  •  Lack of Work Credits (SSDI): You have not worked long or recently enough, generally five of the last ten years.

  •    Substantial Gainful Activity (SGA): You are earning to much money from working.

  •   Excessive Income/Resources (SSI): Your household income or assets exceed strict SSI limits.

  •  Other: Not meeting citizenship/residency requirements, or inability for the SSA to locate you.

 You have the right to appeal this decision, usually starting with a Request for Reconsideration, within 60 days of receiving the notice.

Reasons for Medical Denial

 

  • Insufficient Medical Evidence: Records do not clearly show how your condition limits your ability to work.

  • Condition Not Severe Enough: The SSA determines you can still do your previous work or adjust to other work.

  • Duration Requirement Not Met: The impairment is not expected to last 12 months or result in death.

  • Failure to Follow Treatment: Not following prescribed treatment plans (without a good reason) can lead to denial.

  • Failure to Cooperate: Missing scheduled medical exams or not providing updated records.

Appeals Levels

     Reconsideration: A complete review by someone who did not take part in the first decision.

     Hearing by Administrative Law Judge (ALJ): If denied at  reconsideration, you can request a hearing.

     Appeals Council Review.

     Federal Court Review.
